Returning to the Community: Health Care After Incarceration

This guide, prepared in a joint effort by the Centers for Medicare & Medicaid Services and the US Department of Justice, provides a roadmap to help those returning to the community after incarceration get their health care needs met. Throughout the guide, readers can find tips on how to speak up for their health care needs and learn about ways to connect to free and affordable health care services.

Pioneer Human Services

Pioneer Human Services works to empower justice-involved individuals to overcome adversity and reach their full potential. They offer substance abuse treatment and housing support services. Pioneer Human Services also offers job-readiness training and an entrepreneurship program to help people build their career path.

Living with Conviction

Living with Conviction works to secure economic and racial justice with any marginalized communities, especially formerly incarcerated, though community-driven research, storytelling, and legal empowerment strategies. They provide resources and legal information to those with Legal Financial Obligations (LFOs) to help them understand their rights.
